Direct Deposit Authorization (ACH) and W-9 Submission

Education Minnesota PAC makes electronic payments to all of our members. In lieu of receiving checks from Education Minnesota PAC, members will receive payments into their bank accounts through ACH (Automated Clearing House). E-mail payment notifications are sent to the member at the time a payment is made – this will include information on what is being paid. 

In order to get paid, you will need to complete these fillable PDFs and upload them below:

• ACH authorization form (get the form here)
• W-9 form (get the form here)
• Independent Contractor Agreement (get the form here)

Member canvassers: please submit these ASAP, so you can see a quick payment turnaround after you submit each canvassing shift. Submissions will not be accepted after November 30th.

GOTV Leaders and WALs: please submit ASAP!

A new form must be submitted to Education Minnesota PAC if the bank account number is changed.
