2025 Teacher of the Year candidate form

I attest to the accuracy of all facts in these materials and give permission for them to be used for publication and online. By accepting this nomination, I agree to participate in all pertinent aspects of the Teacher of the Year program and related activities. If selected as Teacher of the Year, I agree to represent the teaching profession and will cooperate in an appropriate manner during such time. I understand that being Minnesota Teacher of the Year may include promotion throughout the year, including one or two days immediately following the announcement. I agree to be available for such promotion and understand that failure to comply may result in disqualification. I also acknowledge that being Teacher of the Year will mean I accept the responsibility, scrutiny and visibility that come with being a public figure. I understand that, if selected, I will serve as Minnesota Teacher of the Year from May 4, 2025 until May 2026.
