Education Minnesota PAC makes electronic payments to all of our members. In lieu of receiving checks from Education Minnesota PAC, members will receive payments into their bank accounts through ACH (Automated Clearing House). E-mail payment notifications are sent to the member at the time a payment is made – this will include information on what is being paid. 

Please complete an ACH authorization form (get the form here) and W-9 form (get the form here) (fillable PDFs) and upload them below. These must be submitted by October 1, 2026 (?).

A new form must be submitted to Education Minnesota PAC if the bank account number is changed.
