Notification of member requesting to pay via credit card

  • This form is designed to be completed by a local leader that knows the remaining amount of dues to collect from a member due to the member missing payroll deductions while on leave during the membership year.
  • The local leader is filling out this form on behalf of the member and the member has requested to pay for their outstanding dues via credit card payment to Education Minnesota for the current membership year.
  • Upon completion of the form, membership staff will contact the member via phone or email to gather the actual credit card information. 
  • Paying via credit card can only be done in a one-time lump sum payment to Education Minnesota 
  • After the credit card information is received and processed, the member and the local will be notified of a successfully processed payment as well as a receipt of payment.
  • If membership staff is not able to connect with the member to gather credit card information, or if the payment is declined, the local is still responsible for the outstanding dues.
